Pig, had worries bout my trans as well. Have metal on metal sound from on throttle to off throttle and vice versa, when shutting off the car it sounds like marbles in a tin can. I also have TRD mount. Really worried me until I replaced the stock flywheel with a new stock unit before Dragonball and the sounds were still there. I can't remember who it was that showed me in France but they had the exact same noises. I've done tons of research and even found on supraforums someone who has replaced every stock drivetrain component multiple times trying to rid the noise with no success. I can't fathom how it came from the factory w/o these noises but seems to be unavoidable the older the cars get. If you do happen to find the culprit, please keep me informed but everything I've read and heard is this is just par for the course.

Alternator is dead or something is preventing it from doing its job. Once the car is on you can actually remove the battery because the alternator powers the car. If the car dies while driving, you can pretty much rule out the battery.

Could still be an air lock in the block from when you refilled it. I know you said you jacked up the car and left the heater and car on while attempting to bleed, but this does not always do the trick. Try squeezing the lower radiator hose while its running and see if that changes the coolant level at all. Aside from that, cap and thermostat are next as stated already.
