In the interest of helping out here, I suggest checking the seal part numbers, my info suggest whats written here is the wrong way round and could end in trouble, I might well be wrong but please check. James -

Hi chaps, its the cracks. Seen it a few times usually goes hand in hand with cars loosing oil through either the turbo or engine. The oil burning generates excessive heat and the cast simply cracks. James

The 2 copper washers were used to space the injector from the rail because the ill fitting seals woudlnt allow the injector to sit into the rail. This allowed for silly sized leaks and a bad spray pattern I would imagine, and was most likely why 1 injector failed whilst being removed and refitted. James

Yes a split turbo to actuator pipe is common and will do this, if not next check is actuator itself and the correct movement of the wastegate. James
