Stock cam's.

It's an NA engine with no modifications now apart from Piggy back and a turbo on the exhaust manifold.

Swapping over for a stock ECU this weekend. Past that, I'll attempt a full removal of the EMU loom to get it running 100% oem ecu only.

I would have thought that you've obviously got a problem on one (or more) of the cylinders if most are showing 200psi but one is as low as 145 . . .

If that is down to bad rings maybe it's as simple as the plug being fouled by oil on that cylinder . . . probably wouldn't happen until the engine is fully warmed up, which would tie in with the misfire not happening when cold

Personally I would get someone to hook up the ignition to a display, so you can SEE that all 6 cylinders are firing correctly. (it will show anything from a dodgy coil to a duff lead or spark plug)

If it's all fine you can at least then rule out any kind of Ignition/HT Fault

(Wish someone would post some suggestions on MY weird 2JZ-GE problem . . . the thread is titled "Strange Acceleration" - hundreds have read it, but no real suggestions as to what it could be!)
