Clutch fork woes

Any idea why the clutch fork on my v161 randomly decides to fall out or any way I can prevent this? I recently changed to a sprung cc six puck clutch and fly, it's pretty juddery to get going and I'm not sure if this juddering is causing the clutch fork to fall out!? Although they just push in there must be something to prevent this.

I didn't enjoy pushing my supra back into the garage this weekend.

 

Any clues?
